QAD is responsible for identifying fraud and/or falsified data based on the information available to them, and/or during the time of their QA Review, and/or during the completion of this Checklist.
A rating with Falsification or Fraudulent Data will be deemed INVALID. The rating must be failed and would need to be corrected and re-submitted (as applicable) to the Registry. The Provider will also initiate their Disciplinary Process.
In the Comments section, please indicate which line item(s) contains the falsified or fraudulent data.
SCORING:
Satisfactory: Field QA Review is free of fraudulent and/or falsified data. Unsatisfactory: Fraudulent and/or falsified data is determined by the QAD during the Field QA Review.
